To help you get ready, Bloomington Township has created a simple 6-Step Appointment Preparation Guide that walks you through the process—from scheduling your appointment to gathering the documents you’ll need.
Before Your Appointment
To help avoid delays, we encourage you to:
- Schedule your appointment early. Contact our office as soon as you realize you may need assistance. Reaching out early gives us the best opportunity to help before your situation becomes more serious.
- Complete your application before your appointment. Applications may be completed electronically or by using a printed copy.
- Review your document checklist carefully. Be sure to gather all documents that apply to your situation. Missing documentation may result in your appointment being rescheduled or your request being delayed.
- Submit documents in advance when possible. Documents may be emailed to our office before your appointment or brought with you on the day of your appointment.
- Attend your scheduled appointment. You’ll receive text message reminders before your appointment. If you no longer need assistance or are unable to attend, please contact us as soon as possible so your appointment time can be made available to another resident.
